Once you reach out, we will ask the referring agency or client to complete two important forms: a Risk Assessment Form and a Referral Form. These forms are essential in helping us gather the necessary information about the individual being referred and assessing whether they are a good fit for our services. The Risk Assessment Form helps us evaluate any potential risks or concerns, while the Referral Form provides details about the client’s background and requirements.

What Happens After Submission?

Once we receive both forms, a member of our experienced team will promptly review the information provided. This initial assessment allows us to determine the best course of action. Based on the details you have supplied, we will make a decision on whether to proceed with a Pre-Assessment.

The pre-assessment is a further evaluation step where we dive deeper into the individual’s circumstances to ensure that we can provide the most appropriate support. If we determine that we cannot accept the application at this stage, we will communicate our decision and explain why the referral was not successful. We want to make sure you and your referral understand our decision and the reasons behind it.

Timely Response

We understand the importance of timely communication, especially when it comes to referrals. That’s why we aim to respond to all referral applications within 48 to 72 hours. Whether we are moving forward with a pre-assessment or need to decline a referral, we ensure that you receive a clear and prompt response.
